The Complete Forms Packet includes the applicant’s eligibility standards for the IUPUI McNair Program. The application forms may be downloaded as an Adobe PDF and filled out by hand or typed on the computer. To download the software use this link http://get.adobe.com/reader
To apply to the McNair Program you must fill out and send in all pages of the application including the required attachments which are outlined in each section below.
Page 1: Program Selection
You may select one or more programs to apply for. Rank the programs by placing a number in the box, with one (1) being your first choice. Please carefully read the descriptions for each program before selecting. If you would like more information about a specific program, additional descriptions can be found online at http://www.crl.iupui.edu or call (317) 274-8880.
Page 2: General Information
In this section please fill in the blanks with corresponding information.
Also, notice this application requires you to attach your academic transcript. This does not need to be an official sealed copy, but in many instances an unofficial copy can be found at your institution’s registrar page. After you have attached your transcript please check the box to confirm that it has been added.
Page 3: Academic Information
Please provide your school history. This part of the application may vary due to your academic status and you may not be able to complete all the sections here. If you need additional space you may attach extra pages.
Page 4: Faculty Mentor Information, References, and Personal Statement
If you have already chosen a faculty mentor please fill in his/her information.
Follow the instructions given on page 4 for listing your references.
This section will also outline the personal statement required. Please carefully read the essay questions and follow the format guidelines given. Then attach this document and include it with your application.
Be sure to sign and date page 4 and follow the submission directions given.
Page 5 and 6: Letter of Recommendation Request Form
All applicants must provide a minimum of two (2) letters of recommendation using the forms on page five and six. These should be from separate individuals who can support and recommend you to participate in a summer research program. The deadline for all recommendation letters is March 1st. Submission directions are at the bottom of page 5 and 6.
Page 7 and 8: Family and Financial Verification Form and U.S. Residency Form
Please carefully read the guided directions given and complete these forms to the best of your ability. Be sure to sign and date at the bottom, note: if you are under 24 years of age you will need a parent’s signature in addition to your own.
Page 9: Checklist
Use this sheet to make sure all portions of this application have been completed.
If selected for the IUPUI McNair Scholars Program, you will need to provide a photocopy of your W-2 form, and federal income tax form 1040, 1040A, or 1040EZ, whichever is appropriate, from last year. This is for documentation purposes.
