Application Instructions

The application forms include the applicant eligibility standards for the IUPUI McNair Program and the URSP/M. The application forms may be downloaded either as Microsoft Word documents or as Acrobat pdf documents. Either type of form may be filled out by hand out using a computer.

You will need to install Adobe Acrobat Reader 5.0 or better on your computer to fill out the pdf documents.

If you do not have a professional version of Adobe Acrobat you must print out your pdf form before closing it or you will lose your work. You cannot save an Acrobat Reader generated form after it is filled out.

To apply to the McNair Program you must fill out and send in the following documents, all available through this web site (except for federal tax forms):

1. U.S. Residency Requirement Form

2. The Student Application Form including:

• Personal and Contact Information
• A Recent Transcript
• Complete Past Educational Data
• A list of three people who can provide recommendations. At least two of the three people should be references that know something about your academic background.
• A Personal Statement (a 1500 word essay about you, your interests, and background and future plans)
• Family and Financial Verification Form

If selected for the IUPUI McNair Scholars program, you will need to provide a photocopy of your W-2 form, and federal income tax form 1040, 1040A, or 1040EZ, whichever is appropriate, from last year, for purposes of documentation.
